This Season a Flu Vaccine is More Important than Ever!

Getting a flu vaccine is more important than ever during 2020-2021 to protect yourself, your family and your community from flu. A flu vaccine this season can also help reduce the burden on our healthcare systems responding to the COVID-19 pandemic and save medical resources for care of COVID-19 patients. The more people vaccinated; the more people protected. Do your part. Get a flu vaccine now!

Instructions:

  1. Review the Vaccine Information Statement
  2. Print the 2020 Influenza Vaccine Consent Form. If you are unable to print, consent forms will be available on site.
  3. Complete the Patient Information, Parent/Legal Guardian Information (if applicable) and Vaccination and Health Related Information sections form using black or blue ink.
  4. Sign and date form.
  5. Bring completed form to the vaccination site. If you are unable to print or complete the form prior to arrival, consent forms will be available on site.

Hours of Operation:
All Centers are open 7:45am to 4:30pm. Phone lines for clinic appointments open at 7:00am Monday through Friday.
