Instructions for Shot Record Only
JCDH utilizes the Alabama Department of Public Health immunization patient registry (ImmPRINT) and JCDH patient records to search for vaccines given. We do not have access to immunization records given out of state. If your shots were given outside of Alabama and you do not have a copy of the vaccines given, contact the health care provider who administered the vaccines. If you are requesting a Certificate of Immunization for school entry, the child must be up to date on required vaccinations to receive the certificate.
To request your or your child’s vaccination record email us at shotrecord@jcdh.org.
Please include the following information in your email:
- First, middle, and last name of person who received the vaccine(s). Please provide the legal name of the person as listed when receiving medical care.
- Date of birth of person who received the vaccine(s)

The record being requested such as:
- Shot record
- Certificate of Immunization for school entry
- Dates of COVID-19 vaccines
- Date of TB Skin test
- Your name and telephone number where you can be reached. We will not be able to process the request if we are unable to reach you by phone or email.
