Birth/Death Certificates
 

Birth Certificates

Vital Records– M-F 7:45am – 4:15pm

Vital Records will issue birth, death, marriage, and divorce certificates to individuals that are entitled to said records and have appropriate ID and payment. When seeking assistance in Vital Records, the individual should comply with the following:
  • Report to top deck in back of the building for parking.

Birth certificates are restricted for 125 years other than to immediate family members.
Immediate family members are:
  • Mother/Father
  • Brother/Sister
  • Son/Daughter
  • Husband/Wife
Birth Certificates:
  • Copy cost is $15.00 for the first copy and $6.00 for each additional copy if ordered at the same time.
  • The person applying has to be 14 years old or older.
  • If further information or forms are needed please call (205) 930-1106.
  • The center for health statistics has birth certificates starting in 1908.
  • Written authorization required for anyone other than an immediate family member.
  • Identification is required to obtain a birth certificate. For a list of accepted forms of identification, please click on the Vital Event Certificate Request Form below.

Death Certificates

Vital Records– M-F 7:45am – 4:15pm

Vital Records will issue birth, death, marriage, and divorce certificates to individuals that are entitled to said records and have appropriate ID and payment. When seeking assistance in Vital Records, the individual should comply with the following:
  • Report to top deck in back of the building for parking.

Death certificates are restricted for 25 years other than to immediate family members.
Immediate family members are:
  • Mother/Father
  • Brother/Sister
  • Son/Daughter
  • Husband/Wife
  • Grandchild, acting for the family or estate
  • Son in law/daughter in law, acting on behalf of his/her spouse
Death Certificates:
  • The informant can apply for a death certificate.
  • Copy cost is $15.00 for the first copy and $6.00 for each additional copy if ordered at the same time.
  • The person applying has to be 14 years old or older.
  • If further information or forms are needed please call (205) 930-1106.
  • The center for health certificates has death certificates starting in 1908.

Hours of Operation:
All Centers are open 7:45am to 4:30pm. Phone lines for clinic appointments open at 7:00am Monday through Friday.
