Child Health Vaccines (shots) Self-Scheduling

This appointment is for returning patients and will allow you to get updated on routine child health vaccines only. You will not see a doctor. Vaccines will be given by Child Health staff.
If you are a new patient and have not received shots at our clinics, please call (205) 588-5234 for registration.
Please bring any record of vaccines given to your child that you may have in your possession. JCDH utilizes the Alabama Department of Public Health Immunization Patient Registry (ImmPRINT) and JCDH medical records to search for vaccines given. We do not have access to vaccines given out of state.
If your child is less than 2 years of age and due for a well-child visit, please consult your primary care physician.
