22-402.11 Wear hair restraints that are designed and worn to effectively keep hair from contacting exposed food, equipment, utensils, linens, and unwrapped single service/single use articles. / The food worker had very long hair that was not restrained.
13-302.12 Label all food or ingredients that are not in the original package with the common name of the food (Except for unmistakably recognized food such as dry pasta) / In the refrigerator, there were 2 unlabeled foods. In the kitchen, there was a large unlabeled bin of confectioners sugar. There were 3 unlabeled covered containers of ingredients.
54-501.114, 4-703.11 Sanitize utensils with quaternary ammonia at a concentration specified by manufacturer for at least 30 seconds. / The food equipment was observed being washed and sanitized manually at the 3-compartment sink. No chemical sanitizer was detected in the 112°F water. / COS; the sanitizing compartment of the sink was cleaned (see, also, item 19 remarks) and chemical sanitizer solution was made in that compartment. The chemical sanitizer was then detected.
14-501.14 Clean the 3-compartment sink. / The inside compartments of the 3-compartment sink were all soiled.
13-304.14. 4-501.114 Provide quaternary ammonia solution at concentration specified by manufacturer for storing cloths used to wipe counters and equipment. / The 2 sanitizer wiping cloth containers were checked and no chemical sanitizer was detected. According to the food worker, soapy water was placed in the sanitizer wiping cloth containers; not sanitizer.
26-501.12 Clean facilities (floors, walls, ceilings, ceiling/wall vent covers, fixtures, etc.) as often as necessary to maintain cleanliness. / There was residue on the floors below the soda box rack, in both stock rooms, behind the ice machine, and in the kitchen.
16-303.11(C) Provide at least 50 foot candles of light in food prep and ware washing areas where employee safety is a factor. / There were 3 large light fixtures not working in the dishwashing, and food prep refrigerator area.
