Community Perspectives on Jefferson County Department of Health Services

The Jefferson County Department of Health will visit selected neighborhoods to hear from residents about JCDH services and community needs.

Survey Schedule September 30 and October 1, 2026 2:00 p.m. to 7:00 p.m.

What Residents Can Expect

Survey teams will walk through selected neighborhoods and invite selected households to participate in a brief, voluntary survey.

Recognize Our Team

JCDH staff will wear uniformed shirts and official photo identification badges showing a name, photo, and the JCDH logo.

Choose Whether to Participate

Participation is voluntary and is limited to household members who are 18 years of age or older.

Share Your Perspective

The survey takes approximately 25 to 30 minutes and will help JCDH better understand resident needs and improve programs and services.

Where Survey Teams May Be Working

Outreach will occur in selected neighborhoods within the following communities and areas:

  • Bessemer
  • Birmingham
  • Fairfield
  • Fultondale, including some unincorporated sections
  • Gardendale
  • Helena
  • Hoover
  • Tarrant
  • Trussville, including some unincorporated sections
  • Unincorporated Jefferson County areas
  • Vestavia Hills and nearby unincorporated areas, including an unincorporated area surrounded by Vestavia Hills
Please Note Only selected neighborhoods and households will be approached. Seeing survey teams walking or going door to door during the scheduled hours is expected.

A Legitimate JCDH Community Survey

The outreach is an official Jefferson County Department of Health activity.

How to Identify Survey Staff
  • Official JCDH photo identification badge
  • Name, photo, and JCDH logo displayed on the badge
  • Uniformed shirt
Your Privacy and Choice Matter
  • Participation is voluntary.
  • The survey will not ask for information such as your education level or place of birth.
  • Information collected will be used to support public health planning and preparedness and to help improve JCDH programs and services.

Why is JCDH conducting this survey?

JCDH is gathering community feedback to better understand resident needs and improve programs and services across Jefferson County.

Do I have to participate?

No. Participation is voluntary. Household members must be 18 years of age or older to participate.

How long will the survey take?

Approximately 25 to 30 minutes.

How can I confirm that a survey team is legitimate?

Look for an official JCDH photo identification badge with the staff member's name, photo, and the JCDH logo. Staff will also wear uniformed shirts.

If you are unsure whether someone is an authorized JCDH survey team member, you do not have to participate. Please call (205) 588-5234 to verify the visit.

Will every household be contacted?

No. Survey teams will invite selected households in selected neighborhoods to participate.

Hours of Operation:
All Centers are open 7:45am to 4:30pm. Phone lines for clinic appointments open at 7:00am Monday through Friday.
