The Department of Health provides outpatient health care for underinsured and uninsured residents.
These centers are open from 7:45 a.m. to 4:30 p.m. M-F (except on Holidays) and accept patients with the following health coverage:
Medicaid, AllKids, Blue Cross and Medicare. However, JCDH does not accept patients who are enrolled in other insurance programs
including patients enrolled in an HMO or with TriCare, except for patients wishing to receive family planning services only.

Adult Health
Primary health care for adults 19 and older is provided at all health centers except Morris.
|
Child Health
Complete, primary health care, including routine exams and sick visits, is provided for children from birth to 18 years of age at all health centers.

Dental Health
Dental care for patients who meet eligibility requirements is provided at Bessemer, Central, Eastern, and Western Health Centers. Click here for more information.

Family Planning
Birth control services, yearly family planning exams, various birth control methods, e.g. pills or Depo-Provera shots, lab work and pregnancy testing. Help in planning when to have a baby and care support from a case manager. JCDH is a Plan First provider. Click here for more information about family planning and the Plan First Medicaid program.

Immunizations
Childhood immunizations are provided at all health centers. Certificates of immunization (blue forms) are provided and required for children enrolling in childcare centers and private and public schools.
Adult immunizations for tetanus, influenza, pneumonia, and other diseases are available at all health centers.

Interpreters
Interpreters for Spanish speaking patients are available at Central, Eastern and
West End Health Centers.

Maternity Care
Maternity care is provided by UAB Dept. of OB/GYN staffs at all health centers. Hours and days of operation may vary at some locations. UAB care coordinators are also on site to enroll women into the Steps Ahead Medicaid Maternity program.

Nutrition/WIC
The Women, Infants, and Children program (WIC) is available for eligible pregnant and breastfeeding women and for infants and children up to five years of age. The program provides free, healthful foods and nutrition information, as well as information on preventing nutrition-related health problems such as diabetes, obesity, and high blood pressure. Pharmacy
Pharmacy services are provided through the Cooper Green Pharmacy program. Prescriptions written by a JCDH provider or CGH provider may be filled at Bessemer, Central, Eastern, and Western health center pharmacies.

Social Services/Case Management
Care Coordinators are available at all centers to provide screening and referral services for patients with social, emotional or financial needs. Medicaid Outstationed workers are located as some clinics to assist with Medicaid applications. To print a Medicaid application click here
